Please be sure you have reviewed the Guidance for Team Assessments before completing this form. In particular, please be sure to review the Analytic Rubric for Teammate Assessments – and be sure to apply that rubric equally to assessing your own performance alongside that of your teammates.

As noted in the course syllabus:

Team Assessments during the course do NOT impact your grade.

However, the Final Team Evaluation at the end of the course will impact your final grade.
About one day after the Team Assessment deadline has passed, you will see the average ratings and anonymised feedback from your teammates (along with your own self-assessment).
Individual Assessments
Please also note that your “Contribution Percentages” must add up to 100 across the team, and percentages also have to be whole numbers (no decimals). You will not be able to submit the form unless these conditions are met.
Private Feedback to Teaching Team
Do you have any specific concerns or issues about your team, and/or any of your teammates, that you want to raise privately with the teaching team?

Your answers here will be treated confidentially, and will NOT be shared with your team members. Rather, the Teaching Team will use your answers to attempt to support you and your team with addressing any issues raised in a constructive manner.
